Overview

HEI founded HealthSignal Partners (HSP) to provide Medicare participants with hands-on, practical support to manage chronic conditions. HSP services range from conducting an annual wellness visit, creating a best practice based care plan, providing disease management technology to supporting customers with services ranging from expediting critical appointments, arranging for medical equipment, reconciling medications to arranging for transportation and meal services. HSP services are considered a Medicare benefit and are provided at either no or little cost to our customers.

Providers

HealthSignal Partners supplements the services rendered by Providers and removes the burden from office staff, effectively decreasing the number of in-bound calls. Time-consuming tasks such as completing forms, submitting referrals and medication refills can be done for the Provider to allow for more focus on the quality of care the patient receives. Patient data is managed and updated regularly, allowing access to the most up-to-date information.

Patients

HealthSignal Partners allows patients to easily access a variety of health resources in one centralized location. These resources are provided by a Health Partner who assists with health management needs and is available 24 hours a day, 7 days a week. Patients receive a personalized care plan that includes assistance with coordination of care and medication adherence.

Home Health/Living Facilities

Provides easy access to health resources in one centralized location that is available 24 hours a day, 7 days a week. Health Partners are also available at any time of day to assist with clinical questions regarding the patient’s care. Better yet, the payer source for HealthSignal Partners CCM service is Medicare, so no revenue is taken away from home health agencies or any other facility.
